- After receiving a test order, we review your patient's plan to see what's covered. If we expect an out-of-pocket cost, we contact your patient to let them know.
- Any patient can apply for our needs-based financial assistance at any time—even before testing is ordered. All that your patients need to do is complete a simple form.

Please confirm your patients' coverage benefits and qualifying medical criteria with individual health plans. Patients may also contact their health plans directly.
Foundation Medicine is considered in-network when we have a contract with a health plan. This contract offers patients more reliable access to our testing and a smoother billing experience. When a health plan provides coverage, but Foundation Medicine is out-of-network, patients may still access this service or treatment, but may pay higher out-of-pocket rates. Foundation Medicine offers a financial assistance program for patients who may have out-of-pocket costs and have financial hardships. Patients can apply online and, if they qualify, won't pay more than $100 per test.
